	Billed Services The following insurance's are accepted with pre-approval:  Workers Comp., Personal Injury $96 per hour Premera Blue Cross, Lifewise, Uniform, Regence, Aetna, Cigna.  All treatment billed to insurance requires a doctor's prescription with a diagnosis for medically necessary treatment.
